Within both our residential and supported living environments, NCG delivers quality outcomes for the people we support and promotes independence and choice through our care plans. Another word for an outcome is a target or goal. Each and every one of us sets outcomes for ourselves on a daily basis. Here at NCG, we support individuals with learning disabilities, mental health needs, autism spectrum conditions, acquired brain injuries and physical disabilities to achieve their own personal outcomes. It is our mission to help the people we support fulfil their potential, develop new skills and live as independently as possible.
One of NCG’s principal values is to empower and inspire others. We encourage every individual to aim high and to live life the way they want to – no goal is too small and no dream is beyond reach. At NCG we believe that outcomes are unique to each individual. A key outcome could be anything from participating in local community events or learning to cook a meal, to enrolling in college or travelling on one’s own.
Our aim is to help the people we support identify a goal and then reach it. We take the time to sit down with each individual to discuss their aspirations and dreams, what they would like to learn, and what they would like to do with their lives. Our support staff then work with the individual to break down the overall outcome into smaller steps and create a clear development plan to work towards. These plans are reviewed at regular intervals and, where appropriate, can lead to individuals moving on to lower acuity services. We support people during the transition period and help them move on from residential care to supported living and, where suitable, to outreach support.
